This self assessment was developed by the North Carolina Division of Public Health (DPH), Committee on Practice and Education (COPE) of the NC Association of Public Health Nurse Administrators (NCAPHNA), the NC Public Health Academy, and the Southeast Public Health Training Center.

You can complete and submit the self assessment online (results will be emailed to you) or by downloading an electronic copy and filling it out by hand. Either way, we can provide you with guidance in developing a learning plan based on the results of your self assessment. If you have any questions, please contact info@ncpublichealthacademy.org.
